The National Health Insurer Report Card released data on Aetna, Anthem Blue Cross Blue Shield, Cigna, Health Care Service Corporation, Humana, Regence, UnitedHealthcare and Medicare. The data was gathered from 380 physician practices between February and March 2012.
Here is the data on timeliness of processed data — the percentage of claims where the first payment was received within zero to 15 days — and accuracy — the percentage of claim lines the payor's allowed amount equal the facility's expected allowed amount.

Aetna:

Timeliness: 14 days
Accuracy: 95.39 percent

Anthem

Timeliness: 7 days
Accuracy: 88.59 percent

Cigna

Timeliness: 7 days
Accuracy: 90.62 percent

HCSC

Timeliness: 6 days
Accuracy: 87.57

Humana

Timeliness: 6 days
Accuracy: 87.36 percent

Regence

Timeliness: 9 days
Accuracy: 89.02 percent

United

Timeliness: 11 days
Accuracy: 98.32 percent

Medicare

Timeliness: 14 days
Accuracy: 99.48 percent
