Rush University Medical Center reports letter mailing mix-up

Practice Management

Chicago-based Rush University Medical Center reported a mailing incident Feb. 11 to HHS.

The incident affected 908 individuals; however, not patient health information was affected.

According to the letter sent to affected patients, Rush discovered the incident Dec. 21, 2018.

Rush found that some letters to patients notifying them of the retirement of a certified nurse practitioner may have included the name of the wrong individual in the greeting/salutation.

Hospitals are required to report all breaches of unsecured protected health information affecting 500 or more individuals to HHS.
