Phoenix-based The Core Institute's management company is re-branding as Healthcare Outcomes Performance Company, or HOPCo.
HOPCo provides services to manage physician practices, clinically integrated networks and service lines within hospitals using health IT platforms and protocols. The company also works with payers on population health initiatives.
"From managing physician practices in new and unique ways to partnering with hospitals and payers, HOPCo's management services are delivering optimum patient care and increased profitability through value creation," said HOPCo Chairman and CEO David Jacofsky, MD. "We will take on full population-based risk in the musculoskeletal space based on our platform's performance."

1. HOPCo aims to help organizations provide better outcomes for orthopedic patients at a lower cost. The company extends cost savings to patients, hospitals and insurers.
2. With proprietary IT tools and decision-support platforms, HOPCo works with physicians and hospitals to drive evidence-based medicine, tracking data and preventing complications in real time. "We're now able to produce validated predictive analytics, assuring patients the best likelihood of a good outcome," Dr. Jacofsky said.
3. The company has already demonstrated success with managing population health with its medical economics team in addition to the claims analytics tools and wellness programs. HOPCo partners with The Core Institute and Phoenix-based Banner Health already.
