The American Association of Orthopaedic Surgeons shared comments to the Center for Medicare and Medicaid Innovation regarding a request for information on developing episode-based payment models.
The AAOS' letter supports physician engagement in the design and implementation of alternative payment models, according to an Aug. 25 news release. The AAOS also urged CMS to consider the effect of interoperability, multipayer alignment of measures and administrative burden on physicians who would want to use alternative payment models.
The AAOS outlined its recommendations and insights on value-based care:
1. Expand alternative payment model episodes beyond 30 days.
2. Only consider voluntary models with participation incentives.
3. Ensure barriers are resolved in future payment models.
4. Find an approach that allows downstream risk sharing.
5. Explore pilot programs for managing chronic and prevalent conditions.
