Double-bundle ACL reconstructions can be used safely if the posterolateral bundle graft is fixed between zero degrees and 15 degrees and the anteromedial graft can be safely fixed up to 75 degrees without restricting knee laxity, according to an article published in The American Journal of Sports Medicine.
Though the range of knee flexation angles a surgeon can use for the AM and PL bundle graft fixation spans between zero degrees and 90 degrees, researchers affiliated with the Steadman Philippon Research Institute at The Steadman Clinic in Vail, Colo., found that tighter range restrictions are necessary.
After biochemically testing 12 cadaveric knees in the intact state, ACL-sectioned state and a randomized order of seven double bundle ACL reconstructive states at multiple graft fixation angles, the researchers found that fixation of the PL graft at 30 degrees and above over-constrained the knee.
