2. Mr. Frazier initially sustained the concussion Feb. 24 after crashing into the left-field wall during a spring training game against the Pittsburgh Pirates.
3. In April, Mr. Frazier was cleared and played 15 big league games before returning to concussion protocol after attempting a diving catch while playing for Moosic, Pa.-based Triple-A Scranton/Wilkes-Barre July 19.
4. The Yankees manager said Mr. Frazier has returned to Tampa, Fla. to continue his rehab assignment with Class-A Advanced Tampa and is unlikely to play in any remaining big league games this season.
