2. Jason J. Chang, MD, a neurological surgery assistant professor at Oregon Health & Science University, performed the surgery and implantation about 77 hours post-injury.
3. Dr. Chang serves as the site's co-study investigator.
4. This patient brings the study's total nationwide enrollment to 15 patients.
